Our 245D Intensive Services provide a higher level of care for individuals who need consistent and specialized assistance. With personalized attention, structured routines, and compassionate support, we ensure safety, stability, and dignity every step of the way.

Individualized Home Supports with Training

This service is for both adults and minors. It combines in-home and community support with skill-building activities. Clients receive help with daily living tasks while also learning the skills needed to stay independent at home and in the community.

Individualized Home Supports with Family Training

This service includes all parts of IHS with training but also adds guidance for family members. Families learn how to support their loved one’s needs, encourage growth, and build long-term success in both home and community life.

Community Participation

  • Learning safe movement in the community (mobility and pedestrian safety).
  • Accessing and using community resources.
  • Understanding and practicing community safety.
  • Building informal support systems and networks.
  • Improving communication skills.
  • Planning leisure, recreation, and social activities.
  • Developing transportation skills.

Household Management

  • Support and guidance to complete daily household tasks.
  • Learning household safety skills.
  • Help with tenancy support and advocacy.

Training, Assistance, and Support

  • Budgeting and money management.
  • Cooking, meal planning, and nutrition.
  • Practicing healthy lifestyle habits.
  • Household chores and basic maintenance (client covers replacement costs if needed).
  • Personal shopping and purchasing needs.

Health, Safety, and Wellness

  • Help arranging health care, activities, services, and appointments.
  • Guidance and support for personal care (not overlapping with Medical Assistance services).
  • Health services support, including building resiliency and wellness strategies.
  • Creating and following personalized health and safety goals.

Adaptive Skills

  • Developing crisis prevention skills.
  • Using positive support strategies.
  • Problem-solving and decision-making.
  • Building sensory and motor skills for independence.
  • Learning strategies for self-sufficiency.
